Information Required for Application Appointments & Map

Please be prepared to bring the following to your appointment:

1-  Proof of Birth for your child* (certified birth certificate preferred, but will accept hospital certificate)

2- Proof of Immunizations for your child*

3- Social Security Card for your child

4- Medical Card for your child (if applicable)

5- Proof of Income for your household for 2011 OR the past 12 months* (this includes any child support statements, wages-  W-2 forms - check stubs -or tax forms, unearned income, unemployment statements, grant/scholarship award letters, SSI/SSD statements, TAFI/TANF, etc.)  
 
6- Written Verification of Disability
(if applicable)


*Applicants without this required information will be asked to reschedule and return with this information at their new appointment.
