Use/Personal Items Rental Report

Due
to Parent/Community Involvement Manager at least once before end of
program year.
|
Please use this form to record all items you have brought into your classroom to
enhance the learning experiences of our Head Start children. These must be
items that were not purchased with Head Start funds and are not considered part
of your permanent classroom inventory. This must be submitted to the
Parent/Community Involvement Manager by Nov. 5th and April 5th so
in-kind can be calculated for these items. Thank you for your help!
